block/loop: Use global lock for ioctl() operation.
syzbot is reporting NULL pointer dereference [1] which is caused by
race condition between ioctl(loop_fd, LOOP_CLR_FD, 0) versus
ioctl(other_loop_fd, LOOP_SET_FD, loop_fd) due to traversing other
loop devices at loop_validate_file() without holding corresponding
lo->lo_ctl_mutex locks.
Since ioctl() request on loop devices is not frequent operation, we don't
need fine grained locking. Let's use global lock in order to allow safe
traversal at loop_validate_file().
Note that syzbot is also reporting circular locking dependency between
bdev->bd_mutex and lo->lo_ctl_mutex [2] which is caused by calling
blkdev_reread_part() with lock held. This patch does not address it.

nvme: add separate poll queue map
Adds support for defining a variable number of poll queues, currently
configurable with the 'poll_queues' module parameter. Defaults to
a single poll queue.
And now we finally have poll support without triggering interrupts!

block: add REQ_HIPRI and inherit it from IOCB_HIPRI
We use IOCB_HIPRI to poll for IO in the caller instead of scheduling.
This information is not available for (or after) IO submission. The
driver may make different queue choices based on the type of IO, so
make the fact that we will poll for this IO known to the lower layers
as well.

nvme: utilize two queue maps, one for reads and one for writes
NVMe does round-robin between queues by default, which means that
sharing a queue map for both reads and writes can be problematic
in terms of read servicing. It's much easier to flood the queue
with writes and reduce the read servicing.
Implement two queue maps, one for reads and one for writes. The
write queue count is configurable through the 'write_queues'
parameter.
By default, we retain the previous behavior of having a single
queue set, shared between reads and writes. Setting 'write_queues'
to a non-zero value will create two queue sets, one for reads and
one for writes, the latter using the configurable number of
queues (hardware queue counts permitting).

blk-mq: improve plug list sorting
Currently we only look at the software queue, but with support
for multiple maps, we should also look at the hardware queue.
This is important since we'll flush out the request list if
either the software queue or hardware queue don't match.
This sorts by software queue first, then hardware queue if
that differs. Finally we sort by request location like before.
This minimizes the flush points per plug list.

blk-mq: cleanup and improve list insertion
It's somewhat strange to have a list insertion function that
relies on the fact that the caller has mapped things correctly.
Pass in the hardware queue directly for insertion, which makes
for a much cleaner interface and implementation.

blk-mq: cache request hardware queue mapping
We call blk_mq_map_queue() a lot, at least two times for each
request per IO, sometimes more. Since we now have an indirect
call as well in that function. cache the mapping so we don't
have to re-call blk_mq_map_queue() for the same request
multiple times.

blk-mq: separate number of hardware queues from nr_cpu_ids
With multiple maps, nr_cpu_ids is no longer the maximum number of
hardware queues we support on a given devices. The initializer of
the tag_set can have set ->nr_hw_queues larger than the available
number of CPUs, since we can exceed that with multiple queue maps.

blk-mq: support multiple hctx maps
Add support for the tag set carrying multiple queue maps, and
for the driver to inform blk-mq how many it wishes to support
through setting set->nr_maps.
This adds an mq_ops helper for drivers that support more than 1
map, mq_ops->rq_flags_to_type(). The function takes request/bio
flags and CPU, and returns a queue map index for that. We then
use the type information in blk_mq_map_queue() to index the map
set.

blk-mq: allow software queue to map to multiple hardware queues
The mapping used to be dependent on just the CPU location, but
now it's a tuple of (type, cpu) instead. This is a prep patch
for allowing a single software queue to map to multiple hardware
queues. No functional changes in this patch.
This changes the software queue count to an unsigned short
to save a bit of space. We can still support 64K-1 CPUs,
which should be enough. Add a check to catch a wrap.

This was used for completion placement for the legacy path,
but for mq we have rq->mq_ctx->cpu for that. Add a helper

block: remove dead elevator code
This removes a bunch of core and elevator related code. On the core
front, we remove anything related to queue running, draining,
initialization, plugging, and congestions. We also kill anything
related to request allocation, merging, retrieval, and completion.
Remove any checking for single queue IO schedulers, as they no
longer exist. This means we can also delete a bunch of code related
to request issue, adding, completion, etc - and all the SQ related
ops and helpers.
Also kill the load_default_modules(), as all that did was provide
for a way to load the default single queue elevator.

ide-disk and ide-cd tested as working just fine, ide-tape and
ide-floppy haven't. But the latter don't require changes, so they
should work without issue.

null_blk: Add conventional zone configuration for zoned support
Allow the creation of conventional zones by adding the zone_nr_conv
configuration attribute. This new attribute is used only for zoned devices
and indicates the number of conventional zones to create. The default value
is 0. Since host-managed zoned block devices must always have at least one
sequential zone, if the value of zone_nr_conv is larger than or equal to
the total number of zones of the device nr_zones, zone_nr_conv is
automatically changed to nr_zones - 1.

HID: fix up .raw_event() documentation
The documentation for the .raw_event() callback says that if the
driver return 1, there will be no further processing of the event,
but this is not true, the actual code in hid-core.c looks like this:

The only return value that has any effect on the processing is
a negative error.
Correct this as it seems to confuse people: I found bogus code in
the Razer out-of-tree driver attempting to return 1 here.
asus_wmi_evaluate_method() is an empty dummy function when CONFIG_ASUS_WMI
is disabled, or not reachable from a built-in device driver. This leads to
a theoretical evaluation of an uninitialized variable that the compiler
complains about, failing to check that the hardcoded return value makes
this an unreachable code path:

Due to what appears to be a copy/paste error, the opening ENTRY()
of cpu_v7_hvc_switch_mm() lacks a matching ENDPROC(), and instead,
the one for cpu_v7_smc_switch_mm() is duplicated.
Given that it is ENDPROC() that emits the Thumb annotation, the
cpu_v7_hvc_switch_mm() routine will be called in ARM mode on a
Thumb2 kernel, resulting in the following splat:

net: bpfilter: fix iptables failure if bpfilter_umh is disabled
When iptables command is executed, ip_{set/get}sockopt() try to upload
bpfilter.ko if bpfilter is enabled. if it couldn't find bpfilter.ko,
command is failed.
bpfilter.ko is generated if CONFIG_BPFILTER_UMH is enabled.
ip_{set/get}sockopt() only checks CONFIG_BPFILTER.
So that if CONFIG_BPFILTER is enabled and CONFIG_BPFILTER_UMH is disabled,
iptables command is always failed.

rtnetlink: restore handling of dumpit return value in rtnl_dump_all()
For non-zero return from dumpit() we should break the loop
in rtnl_dump_all() and return the result. Otherwise, e.g.,
we could get the memory leak in inet6_dump_fib() [1]. The
pointer to the allocated struct fib6_walker there (saved
in cb->args) can be lost, reset on the next iteration.
Fix it by partially restoring the previous behavior before
commit c63586dc9b3e ("net: rtnl_dump_all needs to propagate
error from dumpit function"). The returned error from
dumpit() is still passed further.

arm64: dts: stratix10: fix multicast filtering
On Stratix 10, the EMAC has 256 hash buckets for multicast filtering. This
needs to be specified in DTS, otherwise the stmmac driver defaults to 64
buckets and initializes the filter incorrectly. As a result, e.g. valid
IPv6 multicast traffic ends up being dropped.

compiler: remove __no_sanitize_address_or_inline again
The __no_sanitize_address_or_inline and __no_kasan_or_inline defines
are almost identical. The only difference is that __no_kasan_or_inline
does not have the 'notrace' attribute.
To be able to replace __no_sanitize_address_or_inline with the older
definition, add 'notrace' to __no_kasan_or_inline and change to two
users of __no_sanitize_address_or_inline in the s390 code.
The 'notrace' option is necessary for e.g. the __load_psw_mask function
in arch/s390/include/asm/processor.h. Without the option it is possible
to trace __load_psw_mask which leads to kernel stack overflow.

Fix strpbrk()'s argument order, it must pass acceptable string
in 2nd argument. Note that this can cause a kernel panic where

arm64: dts: renesas: condor: switch from EtherAVB to GEther
The "official" Condor boards have always been wired to mount NFS via
GEther, not EtherAVB -- the boards resoldered for EtherAVB were local
to Cogent Embedded, so we've been having an unpleasant situation where
a "normal" Condor board still can't mount NFS (unless an EtherAVB PHY
extension board is plugged in). Switch from EtherAVB to GEther at last!

genirq/affinity: Add support for allocating interrupt sets
A driver may have a need to allocate multiple sets of MSI/MSI-X interrupts,
and have them appropriately affinitized.
Add support for defining a number of sets in the irq_affinity structure, of
varying sizes, and get each set affinitized correctly across the machine.

genirq/affinity: Pass first vector to __irq_build_affinity_masks()
No functional change.
Prepares for support of allocating and affinitizing sets of interrupts, in
which each set of interrupts needs a full two stage spreading. The first
vector argument is necessary for this so the affinitizing starts from the
first vector of each set.

genirq/affinity: Spread IRQs to all available NUMA nodes
If the number of NUMA nodes exceeds the number of MSI/MSI-X interrupts
which are allocated for a device, the interrupt affinity spreading code
fails to spread them across all nodes.
The reason is, that the spreading code starts from node 0 and continues up
to the number of interrupts requested for allocation. This leaves the nodes
past the last interrupt unused.
This results in interrupt concentration on the first nodes which violates
the assumption of the block layer that all nodes are covered evenly. As a
consequence the NUMA nodes above the number of interrupts are all assigned
to hardware queue 0 and therefore NUMA node 0, which results in bad
performance and has CPU hotplug implications, because queue 0 gets shut
down when the last CPU of node 0 is offlined.
Go over all NUMA nodes and assign them round-robin to all requested
interrupts to solve this.

ARM: dts: imx6sx-sdb: Fix enet phy regulator
Bindings for "fixed-regulator" only explicitly support "gpio" property,
not "gpios". Fix by correcting the property name.
The enet PHYs on imx6sx-sdb needs to be explicitly reset after a power
cycle, this can be handled by the phy-reset-gpios property. Sadly this
is not handled on suspend: the fec driver turns phy-supply off but
doesn't assert phy-reset-gpios again on resume.
Since additional phy-level work is required to support powering off the
phy in suspend fix the problem by just marking the regulator as
"boot-on" "always-on" so that it's never turned off. This behavior is
equivalent to older releases.
Keep the phy-reset-gpios property on fec anyway because it is a correct
description of board design.
This issue was exposed by commit efdfeb079cc3 ("regulator: fixed:
Convert to use GPIO descriptor only") which causes the "gpios" property
to also be parsed. Before that commit the "gpios" property had no
effect, PHY reset was only handled in the the bootloader.

A quoted label reference doesn't expand to the node path and is taken as
a literal string. Dropping the quotes can fix this unless the baudrate
string is appended in which case we have to use the alias.
At least on VF610, the problem was masked by setting the console in
bootargs. Use the alias syntax with baudrate parameter so we can drop
setting the console in bootargs.

bonding/802.3ad: fix link_failure_count tracking
Commit 4d2c0cda07448ea6980f00102dc3964eb25e241c set slave->link to
BOND_LINK_DOWN for 802.3ad bonds whenever invalid speed/duplex values
were read, to fix a problem with slaves getting into weird states, but
in the process, broke tracking of link failures, as going straight to
BOND_LINK_DOWN when a link is indeed down (cable pulled, switch rebooted)
means we broke out of bond_miimon_inspect()'s BOND_LINK_DOWN case because
!link_state was already true, we never incremented commit, and never got
a chance to call bond_miimon_commit(), where slave->link_failure_count
would be incremented. I believe the simple fix here is to mark the slave
as BOND_LINK_FAIL, and let bond_miimon_inspect() transition the link from
_FAIL to either _UP or _DOWN, and in the latter case, we now get proper
incrementing of link_failure_count again.

Since 4.19 the following error in sysfs has appeared when using the
r8169 NIC driver:
$cd /sys/module/realtek/drivers
$ls -l
ls: cannot access 'mdio_bus:RTL8201F 10/100Mbps Ethernet': No such file or directory
[..garbled dir entries follow..]
Apparently the forward slash in "10/100Mbps Ethernet" is interpreted
as directory separator that leads nowhere, and was introduced in commit 513588dd44b ("net: phy: realtek: add RTL8201F phy-id and functions").
Fix this by removing the offending slash in the driver name.
Other drivers in net/phy seem to have the same problem, but I cannot
test/verify them.

This is often because .create_device_attrs lacks a __init

sctp: fix strchange_flags name for Stream Change Event
As defined in rfc6525#section-6.1.3, SCTP_STREAM_CHANGE_DENIED
and SCTP_STREAM_CHANGE_FAILED should be used instead of
SCTP_ASSOC_CHANGE_DENIED and SCTP_ASSOC_CHANGE_FAILED.
To keep the compatibility, fix it by adding two macros.
